摘要： Objective To determine the sensitivity and specificity of circulating cell-free fetal DNA in determining the fetal RHD status and fetal sex. Methods Maternal blood was collected in each trimester of pregnancy from RhD negative nonalloimmunized women. Whole blood was centrifuged, separated into plasma and buffy coat, and frozen at 80 degrees C. DNA analysis was conducted via allele-specific primer extensions for exons 4, 5, and 7 of the RHD gene and for a 37-base pair insertion in exon 4 (RHD pseudogene;psi) three Y-chromosome sequences (SRY, DBY, and TTY2), and an extraction control (TGIFL-like X/Y). RhD serotyping on cord blood and gender assessment of the newborns were entered into a Web-based database. Results One hundred twenty women were enrolled. The median gestational age at the first venipuncture was 12.4 (range: 10.613.9) weeks with 120 samples drawn;118 samples were drawn at 17.6 (1620.9) weeks;and 113 samples at 28.7 (27.933.9) weeks. Overall accuracy for RHD was 99.1%, 99.1%, and 98.1% for each trimester and was 99.1%, 99.1%, and 100% for fetal sex determination. Conclusions Fetal RHD genotyping and sex can be very accurately determined in all three trimesters using circulating cell-free fetal DNA in the maternal circulation. (c) 2012 John Wiley & Sons, Ltd.

摘要： The real estate industry routinely uses specialized information systems for functions, including design, construction, facilities management, brokerage, tax assessment, and utilities. These systems are mature and effective within vertically integrated market segments. However, new questions are reaching across these traditional information silos. For example, buyers may be interested in evaluating the design, energy efficiency characteristics, and operational performance of a commercial building. This requires the integration of information across multiple databases held by different institutions. Today, this type of data integration is difficult to automate and propone to errors due, in part, to the lack of generally accepted building and spaces identifiers. Moving forward, the real estate industry needs a new mechanism to assign identifiers for whole buildings and interior spaces for the purpose of interoperability, data exchange, and integration. This paper describes a systematic process to identify activities occurring at building or within interior spaces to provide a foundation for exchange and interoperability. We demonstrate the application of the approach with a prototype Web application. This concept and demonstration illustrate the elements of a practical interoperability framework that can increase productivity, create new business opportunities, and reduce errors, waste, and redundancy.

摘要： The establishment in recent years of a National Provider Identifier (NPI) offers a new method for counting and categorizing physicians and other health care professionals involved in clinical care. In this paper, I describe how the NPI is assigned, the information collected in association with assigning the NPI, potential ways to enhance information on health professionals through data linkages using the NPI, and how the assessment of the health care workforce could be improved by requiring health care professionals to update their information as a part of maintaining their NPI.

摘要： Patient identification errors are one of the major causes of medication errors. Most medication error studies to date have focused on reporting patient misidentification statistics from case studies, on classifying types of patient identification errors, or on evaluating the impact of technology on the patient identification process, but few have proposed specific strategies or guidelines to decrease patient identification errors. Our study makes three key contributions to the patient identification literature. To better understand the verification of patient identifiers (VPI) process, we first formalize the requirements for this process based on the Joint Commission's national patient safety guidelines. Second, we show the implications of these requirements by applying them to artifacts typically used in medication administration (e.g., patient's statements about their identity, patient's identification band, medication label, and medication order). Third, we evaluate whether nurses comply with these requirements when administering medications using data from clinical simulations. We found that nurses must choose from a considerable number of alternatives to fulfill the Joint Commission guidelines. Despite the number of available alternatives, a small percentage of nurses complied with the requirements for VPI, whether doing so manually or using barcode verification technology. Our findings suggest further study is needed to determine what strategies might improve compliance. © 2013 Copyright Taylor and Francis Group, LLC.
